Transitional Care Services
Transitional care service is designed to bridge the gap between hospital discharge and returning to everyday life. This type of care is particularly beneficial for individuals who have been hospitalized due to surgery, illness, or injury and need extra support during their recovery period.
Rather than leaving patients to fend for themselves once they return home, we ensure that they have:
- Assistance with managing medications
- Following treatment plans
- Coordinating care between different providers
- Accessing community resources.
- Nurses, therapists, social workers, and other professionals make home visits to monitor the patient's progress
- Identify any complications or setbacks
- Help the patient fully transition back to living independently at home.
The overarching goal is to prevent re-admissions by making sure patients are stable, improving, and equipped to manage their health after leaving the hospital.
