Job Summary:

We are seeking a highly motivated and experienced Registered Nurse (RN) with a minimum of 3 years of home healthcare experience and 3 years of supervisory experience to provide exceptional nursing care and leadership to our team. The ideal candidate will possess strong clinical skills, excellent communication abilities, a genuine passion for delivering patient-centered care, and proven leadership abilities. You will be responsible for developing and implementing personalized care plans, administering medications, monitoring patient conditions, providing education and support to clients and their families, and supervising and mentoring home healthcare staff.

Responsibilities:

• Conduct comprehensive initial and ongoing assessments of clients’ health status.
• Develop, implement, and evaluate individualized care plans in collaboration with clients, families, and physicians.
• Provide skilled nursing care, including wound care, medication administration (oral, subcutaneous, intramuscular, and intravenous), and catheter care.
• Monitor and document clients’ vital signs, symptoms, and responses to treatment.
• Educate clients and their families on disease management, medication administration, and self-care techniques.
• Coordinate care with other members of the healthcare team, including physicians, therapists, and home health aides.
• Ensure compliance with all applicable regulations, policies, and procedures.
• Maintain accurate and up-to-date client records using electronic health record (EHR) systems.
• Respond promptly to client emergencies and changes in condition.
• Provide compassionate and respectful care, fostering a positive and supportive environment.
• Supervise, mentor, and evaluate home healthcare staff, including LPNs and Home Health Aides.
• Conduct regular staff meetings and provide ongoing training and support.
• Manage staff scheduling and ensure adequate coverage for all clients.
• Address and resolve staff performance issues and client concerns.
• Participate in the recruitment and onboarding of new staff members.

Qualifications:

• Must reside in or around Flanders, NJ (07836-07930) or Chester, NJ.
• Valid Registered Nurse (RN) license in New Jersey.
• Bachelor of Science in Nursing (BSN) preferred.
• Minimum of 3 years of recent experience as an RN in a home healthcare setting.
• Minimum of 3 years of Supervisory experience.
• Proven ability to develop and implement effective care plans.
• Strong clinical skills and knowledge of home healthcare practices.
• Excellent communication, interpersonal, and organizational skills.
• Ability to work independently and as part of a team.
• Proficiency1 in using electronic health record (EHR) systems.
• Valid driver’s license and reliable transportation.
• Current CPR certification.
• Ability to pass a background check.

Preferred Qualifications:

• Certification in a relevant specialty (e.g., wound care, geriatrics).
• Experience with specific EHR systems.
